Both issues resulted from misconfigurations with the test rather than with Pectra itself

The Ethereum Foundation stated that Sepolia’s issue stemmed from a testnet-specific misconfiguration, not a flaw in Pectra. A similar problem in February’s Holesky test caused a chain split due to validator misconfiguration.
